Please complete the donation form below to send to us with your donated equipment. Remember to include the address of the organization to whom you want your donation directed. Also, make sure that you include a way that we can contact you for any questions. Your notification of donation will be sent to the email address that you provide on this form.

Donation Form (pdf fillable form) . You can complete this form online, and print, but your information will not be saved. You can, however, save the blank form, and fill in later. Use the "print form" button from your browser to print your completed form.
